I am please to announce we are expanding visitation at North Hill Nursing and Rehabilitation Center, LLC. Effective today, July 12, 2021 visitation will begin at 10:30am and end at 6:00pm, to include 7 days a week!!
The visitation guidelines are as follows:
All visitors must screen in and get their temperature checked. All visitors must wear masks in common areas such as Hallways, Lobby area, etc. Please utilize handwashing/hand sanitizer before and after your visit.
If you have not been vaccinated, we ask that you follow CDC guidelines and keep your mask on at all times, do not touch your loved one and use the 6 foot social distance rule. You may not visit or interact with other Residents that are no your loved one. You must take your loved one to a common area such as the lobby, dining room, or outside to visit. Even if you are allowed to go get your loved one from their room you must not stay in the room.
If your loved one has been vaccinated and you are vaccinated you may remove your mask and you are able to touch or have contact with your loved one.
We are so glad to welcome you back into the facility and we want to do everything to keep the Residents and Staff healthy and the facility open so that you can see your loved one!

Do you have beds available?
Bed availability can change on a daily basis. Please contact the Admissions Department for availability.
Do you have Social Workers to assist with discharge planning?
Yes, upon admissions, our social worker will meet with you and discuss the plan of care for the resident. They will follow up on the progress throughout the care plan and will meet with you to assist with discharge planning and initiate any home services desired.
How do I admit a loved one from the hospital?
Inform the social worker at the hospital that you desire admission to North Hill Nursing & Rehab. They will contact and fax the referral to our Admissions Department
How do I admit a loved one from home?
There are certain forms that need to be completed by the primary care physician including a History and Physical, State OBRA screening, admission order and a list of medications. The physician’s office will be familiar with these forms but please contact us; we would love to assist you.
