Inform the social worker at the hospital that you would like admission to North Hill Nursing & Rehab. They will contact our Admissions Department and fax over the referral paperwork on your behalf.

There are certain forms that need to be completed by the primary care physician, including a History and Physical, State OBRA screening, admission order, and a list of current medications. The physician's office will be familiar with these forms — and please don't hesitate to contact us, we would love to assist you through every step of the process.

We recommend bringing personal clothing, toiletries, any personal comfort items such as photos or blankets, insurance cards, and identification documents. Each room has a personal closet and vanity space. Our staff will provide guidance on what is and isn't needed before move-in day.

Yes. Upon admission, our social worker will meet with you to discuss the plan of care for the resident. They will follow the resident's progress throughout their stay, meet with you and the family to assist with discharge planning, and help initiate any home services needed after leaving our facility.

We offer a comprehensive range of therapy services including Physical Therapy, Occupational Therapy, Speech/Language Pathology, Respiratory Therapy, Cognitive Retraining, Dysphagia Therapy, and more. Therapy is available six days per week. Our rehab center is staffed by a dynamic team of licensed physical, occupational, and speech therapists.

Our memory care neighborhood is an intimate, secured 31-bed, all-female unit specifically designed for residents with Alzheimer's and Dementia. The unit features cognitively stimulating activities individualized to each resident's functioning level, an enclosed courtyard for safe outdoor time, and visually stimulating wall murals designed to promote memory recall.

Yes — we have a full-time Respiratory Therapist on staff seven days a week. We provide treatment for tracheostomy management, Asthma, COPD, Pneumonia, CPAP/BiPAP therapy for sleep breathing disorders, Airway management, and more.

Yes. North Hill Nursing and Rehabilitation Center is both Medicare and Medicaid certified. We also participate in several managed care plans, allowing us to assist with most insurance situations. Please contact our Admissions Department to confirm your specific plan is accepted.

We accept Medicare, Medicaid, private insurance, and private pay. Our admissions team will work with you to determine the best payment approach for your situation and can help clarify what your insurance covers before admission.

Visitation is open seven days a week from 10:30am to 6:00pm. All visitors must screen in and have their temperature checked upon arrival. Masks are required in common areas. Please see our Expanded Visitation page for the full guidelines.

We offer both private and semi-private rooms. All rooms include in-room TV, cable, Wi-Fi, microwave, mini-fridge, coffee maker, a personal closet, vanity space, and an in-room restroom. Our Rapid Recovery Unit features 18 spacious private suites with a dedicated private entrance for additional comfort and privacy.

Our activities department organizes multiple programs every day — from games, arts and crafts, music, and social events to cognitively stimulating programs tailored to each resident. Our spacious activity room and enclosed outdoor courtyard provide spaces for both group and individual time throughout the day.
